What documenting looks like
A checklist for the visit type in front of you, worked in order, with room to record what actually happened rather than what was supposed to. You sign; the patient or caregiver counter-signs; it is done before you are back in the car.
If the visit needs a physician's signature, that queue is somebody else's job — not a phone call you have to chase.
